For a long time in pressure ulcer management we only considered protecting our patients while in bed. Then we realized that prolonged sitting for patients who were unable to self reposition while seated, were frail, underweight, or critically ill were at risk for developing or exacerbating pressure ulcers, and needed interventions also. Now pressure redistribution overlays, mattresses, and seat cushions are “married” and working together to protect and treat those patients. End of story ?? Not quite…

IMMERSION THERAPY STARTS IN THE EMERGENCY DEPARTMENT

Come into any hospital with signs and symptoms of a stroke or heart attack and you will quickly be given an aspirin. Come into the Emergency Department with severe pain from a kidney stone and you will most likely be given IV morphine. Come to my community hospital and have a central line or PICC inserted and you will have a passive port protector added, to bathe the connections with alcohol to lessen your risk of a central line related blood stream infection.

PARTNERING WITH PATIENTS: PATIENT PORTALS

Recently I have been tasked by my hospital to help patients sign on to the new patient portal on the hospital’s website. As part of the meaningful use Stage 2 requirement, providers must have “at least five percent of their patients using an online patient portal” to get incentive payments from Medicare and Medicaid. A patient portal is a secure online website that gives patients 24 hour access to personal health information.

TRANSITIONS OF CARE: KEY TO NOT RETURNING

I wonder how many of us consider discharge documentation and instruction giving as just one more thing to do in a busy work day vs. recognizing just how important this one more thing can be.

In a Nov. 11, 2013 article in Nurse.com I read about the transitions of care concept and how important it is to positive patient outcomes and reducing unnecessary hospital readmissions. An example was given about a 68 year old gentleman who was admitted with COPD, DM, atrial fibrillation and heart failure who was placed on a new medication by the consulting cardiologist with the recommendation that the patient have close follow-up upon discharge.
