This month we’re sharing information on the extra importance of pressure mapping and overlays for the critically ill organ transplant patient, and a good reminder of the particular challenges of gluteal cleft skin breakdowns and the need to be aware and vigilant.

I was so glad that I attended the CIOCNS Central Indiana Organization of Clinical Nurse Specialists dinner meeting in June! The speaker was a woman that I am proud to call a colleague, Nancy Verwers MSN, RN, ACNS-BC, a CNS from the IU Health System’s tertiary referral hospital, University Hospital. Nancy works with caregivers who support critically ill patients who are pre and post solid organ transplant (liver, intestine, kidney, pancreas and multivisceral).

THE GLUTEAL CLEFT DILEMMA
Gluteal Cleft Dilemna

As an experienced WOC nurse I have always been challenged by skin breakdown that occurs in the gluteal cleft of some patients. I’m pretty good at identifying moisture related skin breakdown in that area, but distinctly remember several obese patients where their buttock tissue was so firm that a caregiver could not separate the gluteal cleft for skin integrity evaluation or cleansing and drying. As a result damaging pressure ulcers developed. I expect staff nurses and other WOCNs have probably experienced the same patient situation at some time.

WOCN ALGORITHM FOR SUPPORT SURFACE SELECTION AND THE WAFFLE OVERLAY

Increasing awareness of the WOCN Support Surface Algorithm was one of the big topics at the WOCN National Conference earlier this month. The authors of this document and subsequent mobile app were very thorough in their research and comprehensive literature review to produce the algorithm.
