Seamless Continuum of Care Relating to Pressure Ulcers: Protocols are the Glue

This program is designed to address pressure ulcer prevention and management under the new CMS guidelines for long term care and to focus on how these policies will effect the future course of action at all levels of medical care. An individualized patient care plan, in compliance with patient choices, will be examined using protocols based on specific risk factors and recommended good medical practices to ensure a seamless continuum of care. Proper support surface and lower limb device selection will be reviewed as it relates to the individualized care plan.
