The Blueprint of Individualized Care Plans: Avoidable and Unavoidable Pressure Ulcers

Presentation Time: 1 hour, 30 minutes
This presentation focuses on the development of an individualized care plan that can be adapted to all levels of care. Development of this plan is based on specific risk factors that are evaluated in an organized fashion. Specific risk factor care plans are then developed within clinical parameters, utilizing available information, and then blending the clinical recommendations of care with patient choice. Finally, by combining all of the specific risk factor care plans with an appropriate support surface and lower extremity unloading device, an individualized care plan is developed. The individualized care plan is then the basis levels of care in a seamless fashion.
